Leaving the Company
- Notify your manager.
- Review your contact information in Workday.
- Learn more about benefit actions you can take when you leave the company.
- COBRA information will be mailed to your home within 2 weeks of your termination. Email WEX Health or call (866) 451-3399 if you do not receive your COBRA letter.
- If you are enrolled in a Health Savings Account (HSA) or Flexible Spending Account (FSA), HealthEquity will mail you a letter with further information. Contact HealthEquity at (866) 346-5800 with any questions.
- Consider converting your supplemental life insurance to an individual policy. A port and conversion packet will be mailed to you within 10 days of your termination. You have 30 days to convert your policy. Call MetLife at (877) 275-6387 for questions.
- Access free, confidential counseling through the Employee Assistance Program (EAP).
- Exempt employees: Review your time off balances in Workday. Non-exempt employees: all accrued but unused vacation will be included in your final paycheck.
- Log in to your Fidelity account or call 800-835-5095 to have a representative assist you with your beneficiary changes.
